I will show you in brief with screenshots so that you can easily join yourself as a Consultant in the Modicare Network.

First, you will need to open the Modicare website https://www.modicare.com and click on BECOME A CONSULTANT Button at the top. See the below screenshot.

A new screen will be open. Here you can see multiple fields such as.

  1. Sponsor MCA No
  2. Aadhaar Card Number
  3. First Name and Last Name

Fill 87254751 in the Sponsor MCA No field. Then you need to fill your Aadhaar number, first name, and the Last Name, in the second and third fields See below screenshot.

If you are from the North East side and you do not have Aadhaar Card, check this box to continue for further process.

Other users can click on “Authenticate Button“. It will populate(Open) new fields at the bottom. like Personal Details:

  1. Title: In this field put Mr., Miss, and Mrs.
  2. Gender: Select your Gender Male or Female
  3. Email: Fill your email address.
  4. First Name: It will show you pre-filled
  5. Last Name: It will show you pre-filled.
  6. Date of Birth: Select your Date.
  7. Mobile Number: Fill your mobile number without 0 or 91.

Then Fill all Address details.

  1. State: In this field, You need to select your State.
  2. City: Then select your City.
  3. Preferred MSC: Your Preferred MSC will be selected automatically based on your city and state.
  4. Pin Code Number: Here you need to enter your area Pincode.
  5. Address: In this field, enter your address.
  6. PAN CARD NO: Enter your pan card number, if you don’t have Pan Card then select “Applied for PAN CARD option”
  7. Mobile Number: Fill your mobile number without 0 or 91.
  8. Password: Then enter your password of character between 6 to 10, which should contain at least 1 alphabet and no special character is allowed like symbols !@#$%^&*()_+ etc.

Then click on the Submit button. It will take you to the next page, where you have to enter your other details like address proof etc.

You will need to scan your Aadharcard and Voter Id card. You can also take photos on mobile for this. See screenshot.

  1. Address Proof: In this field, You can choose any type form the given options but I prefer to use the Aadharcard option for this. Click On Browse button to attach it.
  2. Document Number: In this field, Enter your Address Proof document number, It always printed on the document.
  3. Photo Identity Proof: For this field, you need to upload any document like PAN CARD/ VOTER ID CARD/ AADHAR CARD.
  4. Document Number: Again, Enter your Photo Identity Proof document number.

Then Click on the Register Button to complete the registration process. After completion you will get your MCA NUMBER on the next congrats page, you will also get this in your mail address.
